Referral Form and informed verbal consent documents for The Multilingual Older Persons COVID-19 Support Line to conduct outbound calls

The Multilingual Older Persons COVID-19 Support Line can conduct outbound calls to older people and their families and carers. The aim of the call would be to inform the consumer or the consumers authorised representative about the Multilingual Older Person’s COVID-19 Support Line, and how it can support the community with COVID-19 updates, aged care services and dementia supports.

In order for a referral to be made, informed verbal consent must given by the consumer or the consumer’s authorised representative. A copy of the informed verbal consent documents to be read and provided to consumer or consumer’s authorised representative are available below for download.
